2026 Fantasy Outlook
There were some good things to take away from Alcantara's first full season back from Tommy John surgery. He made 31 starts and threw 174.2 innings, which probably exceeded the expectations of even the most optimistic prognosticators. Alcantara's velocity was also essentially all the way back, which, we were reminded by Spencer Strider, isn't always a given. The results, unfortunately, were not there for Alcantara for much of the season. He posted an 8.47 ERA and 1.67 WHIP in the first two months, then was up-and-down for his next 12 starts. Alcantara's final eight outings, though, looked like vintage Sandy, as he held a 2.68 ERA, 0.86 WHIP and 52:10 K:BB over 53.2 frames. The strong finish offers hope that Alcantara can return to peak form consistently in 2026, but it would be risky to draft him as if that's a given. Read Past Outlooks
Gives up three homers in loss
Alcantara (13-10) took the loss against the Mets on Tuesday, allowing seven runs on seven hits and four walks while striking out one batter in 4.1 innings.
Analysis
Alcantara put the Marlins in a large hole early, with four of the six runs he gave up through the first three innings coming on home runs from Francisco Lindor, Juan Soto and Brett Baty. It's the fourth time this season (and second time since the All-Star break) that Alcantara has given up three home runs in a start, and this marks the fourth time this year that he's given up at least seven earned runs in an outing. He has an 0-4 record over his last six starts with a 4.95 ERA and 1.40 WHIP over 36.1 innings. Alcantara will look to get back on track next week against the Diamondbacks.

Alcantara spent the year rehabbing from Tommy John surgery undergone in October 2023. The timing of the procedure limited the right-hander to bullpen sessions, though he began to face live hitters in mid-September. All the progress reports were encouraging, but when he takes the hill in the Grapefruit League, it will be more than16 months since Alcantara threw a ball in anger. Typically, the exercises involved in Tommy John rehab strengthen the area, allowing the pitcher to match, and often add, velocity. The concern is always command and control, which were Alcantara's main assets before the injury. Even with a 98-mph fastball, Alcantara's strikeout rate was above average, but not elite. Workload will also be an issue. Rostering Alcantara is a matter of risk tolerance and level of reliance. It's best to treat him as a luxury item and not a staff fixture.

Latest Fantasy Rumors
Unlikely to be traded
The Marlins aren't expected to trade Alcantara before the Aug. 3 deadline, according to Craig Mish of Marlins.tv.
Analysis
Even though rival clubs are expected to kick the tires on Alcantara's availability before the Aug. 3 deadline, the reality is that the Marlins aren't expected to listen to offers for their ace. Even though he hasn't been the dominant pitcher he was in the past, he still owns a solid 4.01 ERA across an MLB-leading 143.2 innings in 2026. The 30-year-old is under contract until the end of the season; however, Miami has a club option for 2027 worth $21 million, so the Marlins aren't likely to give Alcantara away when he's still under control for another year.
