Hazard scored 19 goals and dished out 11 assists in all competitions this season, which was his most productive since moving to Chelsea.

The Belgian was in excellent form all season, as he never went more than two matches without factoring into a goal and even won 14 Man of the Match awards. Hazard is firmly in manager Jose Mourinho's lont-term plans, as he is under contract until 2020.

Hazard is in the starting lineup for Sunday's season finale against Sunderland.

Hazard had oral surgery this week and was doubtful for the match, but he'll take his regular spot in the starting lineup for the 38th time this season against a weak Sunderland side that has already clinched their spot in the Premier League for next season.

Hazard could miss the Premier League season finale Sunday against Sunderland after undergoing dental surgery this week, manager Jose Mourinho said Friday.

Hazard, the PFA Player of the Year, has started all 37 games so far this season, so fantasy owners who rode him this far can't fault him for missing the last match which has no importance on either side of the pitch. It's unclear who could replace him, though it certainly opens a spot up for Juan Cuadrado, who has been sparingly used since being acquired in January.

Hazard won the PFA Player of the Year Award on Sunday.

Hazard was widely believed to be the favorite ahead of finalists Diego Costa, Alexis Sanchez, David de Gea, Philippe Coutinho and Harry Kane, the latter of whom won the PFA Young Player of the Year Award. Hazard co-leads the Premier League in goals by a midfielder (13), while his eight assists trail only Cesc Fabregas, Gylfi Sigurdsson, Angel Di Maria and Leighton Baines. He is arguably the most important piece of the Chelsea attack, one that has them just six points away from clinching the Premier League title.

Hazard took a team-high four shots and scored in the 38th minute of Chelsea's 1-0 win over Manchester United on Saturday.

With Diego Costa (hamstring) still sidelined, Hazard has taken the attack on his shoulders. He's been excellent of late, scoring four goals in his last six games, with two assists over that span. Chelsea will all but lock up the Premier League title with a win against Arsenal at the Emirates next Saturday.

Hazard assisted on Cesc Fabregas' 88th-minute goal during Chelsea's 1-0 win at QPR on Sunday.

The assist was Hazard's eighth of the season, which puts him behind only Fabregas, Manchester United's Angel di Maria and Swansea's Gylfi Sigurdsson in the Prem.

Hazard scored from the penalty spot in the 39th minute and then assisted on Loic Remy's 62nd-minute goal during Chelsea's 2-1 win over Stoke City on Saturday.

Hazard, who has scored three goals in his last four Premier League matches, now boasts an even dozen goals during league play this season, which he's coupled with seven assists. It's been a dominant season for the Belgian, who may have to step up even further on the offensive end with Diego Costa sidelined for multiple weeks after aggravating a hamstring injury Saturday.

Hazard has been playing very well of late, scoring goals in three of his last four games in all competitions. He remains an elite fantasy options in nearly all formats and is a great play against Stoke.
